Based on the Home and Community-Based Services (HCBS) model, which is tailored to meet the unique needs and preferences of each individual, Columbus conducts person-centered plans to establish the needs and desires of our clients. This person-centered approach ensures our clients receive the care and support they need to live on their terms. HCBS improves the quality of life for our clients by providing them with access to necessary services and supports while discovering new opportunities for work and community-based activities. This approach has opened many doors and experiences for our clients as they discover new interests and participate in weekly plans for new adventures.

The Columbus Day Activity program offers supervised care for individuals who want to participate in structured activities and whose families and caretakers seeking community-based respite care

The multi-purpose facility that houses the program was expanded in 2012, reflecting Columbus’s commitment to providing a safe and accessible environment for individuals with disabilities. There is a balance of therapeutic, recreational, and leisure activities while encouraging social interaction and integration in the community.

Our Day Activity Program currently serves an average of 75 individuals a day, with a 3:1 client-to-staff ratio. Program services are available Monday through Friday, from 6:00 a.m. to 4:30
